1. Domestic Worker PaySlip

The Basic Conditions of Employment Act requires employers to give their workers a pay slip.

What is The Purpose of This Form?

To give workers a record of their pay.

Who Fills in This Form?

The employer.

Instructions

Employers must give this pay slip to workers -
  • at the workplace or a place agreed by workers; and
  • during the workers' normal working hour

2. Wages Register

The Basic Conditions of Employment Act requires that employers keep records of time worked and pay received by workers.

What is The Purpose of This Form?

This form provides a template for employee records showing time worked and pay received.

Who Fills in This Form?

The employer

Instructions

Employers must keep this form for 3 years after the last entry.
 
Employers who keep this record do not have to keep any other records about working time and pay of their workers.
 
Workers have the right to inspect the records.

3. Attendance Register

The Basic Conditions of Employment Act requires employers to keep record of the attendance of their workers.

What is The Purpose of This Form?

To keep a record of attendance.

Who Fills in This Form?

The worker, or if the worker is unable, the employer.

Instructions

Employers must keep this form for three years after the last entry.
 
If employers keep this record, they do not have to keep any other records about their workers' working time or pay.

4. Certificate of Service

The Basic Conditions of Employment Act requires emloyers to give their workers a Certificate of Servive when their employment is ended.

What is The Purpose of This Form?

To serve as proof that the worker worked for the employer.

Who Fills in This Form?

The employer.

Instructions

Employers must give this form to workers when they end their employment.
 
The reason for ending the employment must only be completed if workers ask for it.
